Add Trusted Origins to CSRF Handler (#2301)
Enables the user to add addresses that they expect incoming connections
to come from. Helpful in cases where TLS termination is handled by a
load balancer upstream, rather than the application itself.

Initial Implementation of a Password Policy (#1867)
This PR adds the initial work to implement a password policy as defined in #1538.

Specifically, this implements the following

* Rate limiting for the login handler
* Implementing the ability for system admins to require a user to reset their password
* Implementing a password policy that requires passwords to be a minimum of 8 characters
* Removes the default password (gophish) for admin users to instead have the password randomly generated when Gophish first starts up
* Adds a password strength meter when choosing a new password

Jordan Wright 07b46d226a Updated the TLS configuration.
This commit removes support for the TLS_ECDHE_ECDSA_WITH_AES_256_CBC_SHA and TLS_ECDHE_ECDSA_WITH_AES_128_CBC_SHA ciphers. It also removes support for CurveP384. This is to match up with recommendations given by Cloudflare [0], Mozilla (the "Intermediate" compatibility) [1], and referencing the default ciphers in Caddy [2].

Jordan Wright 79e680e675 Updates the tls.Config of the phishing and admin servers to support TLS 1.2 as the minimum TLS version. This addresses #1691 and #1689.
I am making this change since Microsoft, Google, and Apple have all chosen to deprecate TLS 1.0 and TLS 1.1 in early 2020. In late 2018, the companies recorded that less than 1.4 percent (max) of their connections used < TLS 1.2.

Implement User Management API (#1473)
This implements the first pass for a user management API allowing users with the `ModifySystem` permission to create, modify, and delete users. In addition to this, any user is able to use the API to view or modify their own account information.
